Completing the Registration Form:
Enter your general information on page 1. All fields are required.
On page 2, enter information about your group's participation in the Health & Wellness Kickback. All fields are required.
Two selections under "Event Sessions" determine your registration fee. We base these on attributes of your organization and the footprint you wish to have at the Kickback. You will select from the following:
For group type:
- Individual/Small Organization - Free
- Large Organization (100+ employees) - $100
- Fortune 1000 Company - $500
For booth type:
- 10x10 uncovered | no chairs, no table, no tent, just space - $100
- 10x10 uncovered | 2 chairs, 1 table, no tent - $175
- 10x10 covered - middle booth | no chairs, no table, 1 tent - $325
- 10x10 covered - middle booth | 2 chairs, 1 table, 1 tent - $375
- 10x10 covered - corner booth | no chairs. no table, 1 tent - $360
- 10x10 covered - corner booth | 2 chairs, 1 table, 1 tent - $410
- Two 10x10s covered | 4 chairs, 2 tables, 2 tents - $675
Additonal Chairs $15 each, Additional Tables $30 each, Additional Tents $125 each
